A travel health appointment includes:
- A discussion of recommended prophylactic medications (e.g., anti-malarial drugs, antidiarrheal, altitude sickness, etc.)

GCU students who are registered for classes at GCU do not need health insurance to access health care at Canyon Health and Wellness Clinic. However, we strongly urge every student to maintain adequate health insurance to cover unexpected medical expenses.
Consider purchasing our Student Health Plan offered by United Healthcare if your health insurance plan has:
- A high deductible
- No out-of-network benefits
- Emergency only coverage
- Limited benefits for specialty visits such as counseling, physical therapy or nutrition counseling
Learn About Providers
The Canyon Health and Wellness Clinic is proudly managed by a board-certified physician and nurse practitioners who can prescribe medications, administer physical exams, treat illnesses and provide holistic individualized care to all of our students. Our medical providers are board-certified in adult, family or women's health. They complete ongoing continuing education each year to stay informed about best practices for patient care.
Once a patient turns 18, even if they are on their parent's or spouse's insurance, healthcare providers and staff are unable to speak directly with family members (including parents) unless the patient signs a HIPAA release form specifically listing that information can be released to specific family members. This form is included for patients to fill out upon their first visit at the clinic as part of their new patient paperwork.
The options for payment of services include the following:
Fee for service: If you do not have insurance or the student health center is not contracted with your insurance carrier, you will be charged a $50.00 visit fee. There could be additional charges for lab tests, X-Rays, medical procedures, supplies or medication.
Student Health Insurance plan (UHCSR): Canyon Health and Wellness will file claims with the Student Health Insurance carrier, which processes the claim for payment according to your plan benefits. Patient will be charged their office visit copay at time of service. For more information on this optional plan see: https://www.uhcsr.com/GCU
Commercial Health Plans: Most major insurance plans are accepted. If Canyon Health and Wellness Clinic is a Participating Provider with your commercial plan, we will file claims with your insurance carrier, which processes the claim for payment according to your plan benefits. The patient will be charged their office visit copay at time of service. You will receive a billing statement via mail for any charges that your insurance carrier does not pay.

Health Forms
Before your appointment at the Canyon Health and Wellness Clinic, you may want to print, fill out and review the forms below. This will help expedite the check-in process and help ensure that your appointment goes smoothly. Students can access all forms online through the student health portal. Staff and faculty, please print, fill out and sign forms prior to your appointment.
Fill out the Patient Registration Form with your basic contact information to help our staff assist you. You may complete this form prior to your appointment.
As part of your appointment, you will be asked to fill out a Medical Release Form. You can also print and fill out this release prior to your appointment.
This form needs to be filled out by parents for students under the age of 18. If you are a minor, have your parents fill out the form to receive treatment from our clinic.
By providing your health history on the Health & History Form, your medical provider can learn more about you and how best to treat your symptoms.
If you will be living on campus, you must fill out the Immunization Form and submit through your student portal.

Nutrition Counseling Services
Individual nutrition counseling services are available to GCU students through the Canyon Health and Wellness Clinic. Our goal is to help students live their healthiest lives by tailoring their diets to meet their personal nutrition needs. A few reasons to seek nutrition counseling include:
- General health and wellness
- Food allergy or intolerance
- Eating disorders or disordered eating
- Special diets (vegetarian, vegan, gluten-free, etc.)
- Weight management
- Digestive disorders
- Diabetes
- High blood pressure
- High cholesterol
Nutrition counseling is provided by a registered dietitian (RD). RDs are food and nutrition experts who can translate the science of nutrition into practical solutions for healthy living. RDs use their nutrition expertise to help individuals make positive lifestyle changes. Some services an RD can provide include:
- Setting realistic nutrition goals and developing strategies for achieving them
- Maintaining a healthy, well-balanced eating pattern
- Planning nutritious meals with limited financial resources
- Making healthy choices at on-campus eateries
- Identifying signs and symptoms of disordered eating
- Making appropriate referrals

The Lopes Orthopedic Clinic (LOC) at GCU is a collaborative effort between the Canyon Health and Wellness Center and the College of Nursing and Health Care Professions’ athletic training program (ATP). This clinic’s mission is to minimize the physical, emotional and financial burdens associated with injuries and/or illnesses incurred during physical activity through the delivery of high-quality rehabilitation services.
Certified athletic trainers (ATC) are healthcare professionals (HCP) who collaborate with physicians and other healthcare professionals to optimize patients who are both physically active and sedentary. Athletic training encompasses the prevention, diagnosis and intervention of emergency, acute and chronic medical conditions involving impairment, functional limitations and disabilities.

To continue your medications over the summer, speak with your healthcare provider or send a secure message via the student health portal to discuss options and receive help transferring your prescription to your hometown.
Yes, we can provide a note for the date you are seen in the clinic, and depending on your illness, we may extend the time on a case-to-case basis.
Care in the LOC will be provided by licensed and certified athletic trainers. Additionally, GCU offers a four-year accredited athletic training program. Therefore, students may also be present in the clinic assisting with care under the direct supervision of one of GCU's certified athletic trainers.
