Learn About Providers

The Canyon Health and Wellness Clinic is proudly managed by a board-certified physician and nurse practitioners who can prescribe medications, administer physical exams, treat illnesses and provide holistic individualized care to all of our students. Our medical providers are board-certified in adult, family or women's health. They complete ongoing continuing education each year to stay informed about best practices for patient care.

Once a patient turns 18, even if they are on their parent's or spouse's insurance, healthcare providers and staff are unable to speak directly with family members (including parents) unless the patient signs a HIPAA release form specifically listing that information can be released to specific family members. This form is included for patients to fill out upon their first visit at the clinic as part of their new patient paperwork.

Health Forms

Before your appointment at the Canyon Health and Wellness Clinic, you may want to print, fill out and review the forms below. This will help expedite the check-in process and help ensure that your appointment goes smoothly.

Patient Registration Form

Fill out the Patient Registration Form with your basic contact information to help our staff assist you. You may complete this form prior to your appointment.

Medical Release

As part of your appointment, you will be asked to fill out a Medical Release Form. You can also print and fill out this release prior to your appointment.

Parent Authorization

This form needs to be filled out by parents for students under the age of 18. If you are a minor, have your parents fill out the form to receive treatment from our clinic.

Personal and Health History

By providing your health history on the Health & History Form, your medical provider can learn more about you and how best to treat your symptoms.

Immunization Confirmation/Exception

If you will be living on campus, you must fill out the Immunization Form and submit through your student portal. 

Privacy Practices

The Canyon Health and Wellness Notice of Privacy Practices describes how medical information about you may be used and disclosed.

Nutrition Counseling Services

Individual nutrition counseling services are available to GCU students through the Canyon Health and Wellness Clinic. Our goal is to help students live their healthiest lives by tailoring their diets to meet their personal nutrition needs. A few reasons to seek nutrition counseling include:

  • General health and wellness
  • Food allergy or intolerance
  • Eating disorders or disordered eating
  • Special diets (vegetarian, vegan, gluten-free, etc.)
  • Weight management
  • Digestive disorders
  • Diabetes
  • High blood pressure
  • High cholesterol

Nutrition counseling is provided by a registered dietitian (RD). RDs are food and nutrition experts who can translate the science of nutrition into practical solutions for healthy living. RDs use their nutrition expertise to help individuals make positive lifestyle changes. Some services an RD can provide include:

  • Setting realistic nutrition goals and developing strategies for achieving them
  • Maintaining a healthy, well-balanced eating pattern
  • Planning nutritious meals with limited financial resources
  • Making healthy choices at on-campus eateries
  • Identifying signs and symptoms of disordered eating
  • Making appropriate referrals

Immunization Requirements

Requirement for GCU Residents: Effective August 2016

To ensure the health of the student population living on campus, GCU requires you to submit documentation that you have received the measles, mumps and rubella (MMR) vaccine or proof of immunity and meningococcal A vaccine prior to registering for student housing.

Acceptable evidence of MMR immunization is defined as one of the following:

  • Written documentation of two MMR vaccines (the first dose after the first birthday and 28 days or more separating the first and the second dose)
  • Laboratory evidence of immunity (titer)
  • Born prior to Jan. 1, 1957 (exempt)
  • Documentation of physician diagnosed disease(s) (not acceptable for rubella)

If there is not acceptable evidence of immunity, then two doses of MMR, not less than 28 days apart, will be required for admission. The MMR vaccine is not recommended for women who are pregnant or women who anticipate getting pregnant within one month of the vaccine.

Acceptable evidence of Meningococcal A immunization is defined as one of the following:

  • MCV4 or MenACWY (Menveo, Menactra)
    • Effective Nov. 1, 2017
    • Last dose must be given on or after the 16th birthday

Student may waive any of the three immunizations and live on campus by completing the Immunization Exemption Form (found under Health Forms on this page). Please ensure you read the form completely prior to signing. Immunization information can be uploaded via your student checklist.

  • Tetanus-Diphtheria-Pertussis booster
  • Meningococcal vaccine*
  • HPV vaccine series (for women 26 years old or less)
  • Hepatitis B vaccine series of three shots

*Required for previously unvaccinated college freshmen living on GCU's campus, beginning in August 2016.

It is also recommended that a TB (tuberculosis) skin test is obtained annually.

  • Many of these immunizations can be obtained at the Canyon Health and Wellness Clinic. 
  • Submit immunization records or the Immunization Exemption Form via your student checklist located in your Student Portal.

It is your responsibility to verify with the Canyon Health and Wellness Clinic or your SSC that appropriate documentation has been submitted.

The Grand Canyon University Orthopedic Clinic

The Lopes Orthopedic Clinic (LOC) at Grand Canyon University is a collaborative effort between the Canyon Health and Wellness Center and the College of Nursing and Health Care Professions’ athletic training program (ATP). This clinic’s mission is to minimize the physical, emotional and financial burdens associated with injuries and/or illnesses incurred during physical activity through the delivery of high-quality rehabilitation services.

Certified athletic trainers (ATC) are healthcare professionals (HCP) who collaborate with physicians and other HCPs to optimize patients who are both physically active and sedentary. Athletic training encompasses the prevention, diagnosis and intervention of emergency, acute and chronic medical conditions involving impairment, functional limitations and disabilities.

Care in the LOC will be provided by licensed and certified athletic trainers. Additionally, GCU offers a four-year accredited athletic training program. Therefore, students may also be present in the clinic assisting with care under the direct supervision of one of GCU's certified athletic trainers.

Services for the LOC are offered to all GCU students, staff and faculty experiencing pain from injury or limitations in physical activity.
